The confusion here comes from the term "Secondary" used by Blizzard.
Dex and Dex Secondary are in the same affix group, so you can't get both at once. Some items are eligible only for Dex, some only for Dex Secondary, and some for both (eligible, but never more than one on an item).
However, all the combo stats (e.g., DexVit, DexStr, etc) are in their own affix groups so you can get Dex + DexVit (or DexVit Secondary) on an item.

You are correct. The Dex value of 77 either came from "Dex 9" (alvl 35 with a range of 72-81) or from "Dex 13 Secondary" (alvl 59 with a range of 72-77).
Also note that there is no guarantee that an item will have a "main" stat at all. All the affixes are rolled randomly (within those that are eligible given the item level).
